Leg Extension: Safe or Dangerous?

Is the leg extension safe for your knees? Evidence-based answer on this controversial exercise.

The leg extension has been called the most dangerous gym exercise for years. But is this reputation deserved? The short answer: for most healthy people, leg extensions are safe and effective when done correctly.

The Criticism

Critics argue that leg extensions:

  • Place excessive shear force on the ACL
  • Create unnatural knee stress
  • Are not functional

The Evidence

ACL Stress

Studies show ACL stress during leg extensions is highest at 0-30 degrees of knee flexion (near full extension). However, the actual forces are within what healthy ACLs can handle. The concern is mainly for people with existing ACL injuries or post-surgical patients.

Knee Pain

For healthy knees, leg extensions do not cause knee problems. For some people with existing patellofemoral pain (pain behind the kneecap), full-range leg extensions may aggravate symptoms - but partial-range extensions (avoiding the last 30 degrees) are usually well-tolerated.

Muscle Building

Leg extensions are excellent for quad isolation:

  • High EMG activity in the rectus femoris
  • Constant tension throughout the range
  • Easy to take to failure safely (no risk of getting stuck under a bar)

Safe Leg Extension Protocol

  • Avoid lockout if you have any knee discomfort - stop 10-15 degrees short of full extension
  • Use moderate weight - this is an isolation exercise, not a strength test
  • Control the eccentric - 3 seconds down, do not let the weight drop
  • Full range if pain-free - healthy knees can handle full range
  • Higher reps - 12-20 reps works best for this exercise

Who Should Avoid Leg Extensions

  • Post-ACL surgery patients (without physiotherapist clearance)
  • People with active patellar tendinopathy
  • Anyone with current acute knee injury

For everyone else - including most Malaysian gym-goers - leg extensions are a safe, effective quad exercise.
