Muscle Soreness and DOMS: What It Means and What to Do
DOMS explained - why you get sore after workouts, whether it means growth, and how to train through it. Evidence-based recovery strategies.
You crushed leg day. Two days later, walking down stairs feels like a punishment. Sitting on the toilet requires careful negotiation. This is DOMS - delayed onset muscle soreness - and it is the most misunderstood phenomenon in fitness.
What Is DOMS?
DOMS is muscle pain and stiffness that develops 12-72 hours after unaccustomed or intense exercise. It typically peaks at 24-48 hours post-exercise and resolves within 3-7 days.
The soreness comes from micro-damage to muscle fibres, primarily caused by eccentric (lengthening) contractions. Walking downhill, lowering weights slowly, and the landing phase of jumping all emphasise eccentric loading.
Does Soreness Equal Growth?
This is the most common misconception. Soreness does NOT reliably indicate muscle growth. You can build significant muscle without ever being sore, and you can be extremely sore without any meaningful growth stimulus.
Soreness primarily indicates novelty - your body encountered a stimulus it was not prepared for. The first time you do Romanian deadlifts, you will be sore for days. After doing them for 6 weeks, the soreness disappears even though the exercise is still building muscle.
Using soreness as your metric for a good workout leads to counterproductive training choices - constantly changing exercises, always training to failure, and avoiding consistency - all of which actually impair long-term progress.
What Actually Causes DOMS
Eccentric-heavy movements cause more DOMS than concentric-heavy movements. Running downhill creates more soreness than running uphill. Lowering a heavy dumbbell creates more soreness than lifting it.
Novel exercises that your body has not adapted to. Any new exercise will cause DOMS initially, regardless of intensity.
Returning after a break. Two weeks off creates enough deconditioning that your regular routine causes significant soreness upon return.
Excessive volume. Going from 3 sets to 6 sets overnight substantially increases DOMS.
How to Manage DOMS
Keep moving
Light activity increases blood flow to sore muscles, which helps clear metabolic waste products and delivers nutrients for repair. Walking, easy cycling, swimming, or light stretching - anything that moves blood through the affected muscles without creating additional damage.
The worst thing you can do is sit completely still for three days. You will feel stiffer and more sore.
Stay hydrated
Dehydration amplifies the perception of soreness. In Malaysia's climate, drinking 2-3 litres of water daily is a baseline. During heavy training periods, increase to 3-4 litres.
Sleep
Muscle repair occurs primarily during sleep. Growth hormone, which drives tissue repair, is released in pulses during deep sleep. Aim for 7-9 hours. Compromised sleep extends recovery time.
Nutrition
Adequate protein (1.6-2.2 g/kg bodyweight) provides the amino acids needed for muscle repair. Anti-inflammatory foods - berries, fatty fish, turmeric, ginger - may modestly reduce inflammation. But do not chase aggressive anti-inflammatory strategies (ice baths, NSAIDs) immediately after training - the inflammation is part of the adaptation process.
Time
DOMS resolves on its own within 3-7 days. There is no shortcut that dramatically speeds this up. All recovery strategies provide marginal improvement - the body heals at the speed it heals.
Should You Train Through DOMS?
Yes, usually. Training a sore muscle group at moderate intensity actually accelerates recovery by increasing blood flow. This is called the "repeated bout effect" - the second session often reduces soreness faster than rest alone.
However, do not train a muscle group at maximum intensity while severely sore. Reduced range of motion from soreness changes your movement patterns, which increases injury risk under heavy load. If your quads are so sore you cannot squat to depth, train upper body or do a lighter lower body session.
DOMS vs Injury Pain
DOMS: Generalised muscle ache that is symmetrical (both legs sore, not just one), feels like tightness and tenderness, improves with movement, and gradually resolves over days.
Injury: Sharp, localised pain, often on one side only, may worsen with specific movements, may not improve with light activity, and does not follow the typical DOMS timeline.
If pain is sharp, localised, sudden in onset, or has not improved within a week, see a physiotherapist or doctor. DOMS is uncomfortable. Injury pain is concerning.
Reducing DOMS Long-Term
The best strategy is gradual progression. Increase training volume by no more than 10-20% per week. Introduce new exercises one at a time rather than changing your entire programme at once. Maintain consistency - regular training creates the repeated bout effect that reduces DOMS over time.
Your body adapts. The soreness you feel in week 1 of a new programme will be significantly less by week 4. This adaptation does not mean the exercise is no longer effective - it means your body has become efficient at handling the stimulus.
