Why You Get Muscle Cramps During Exercise and How to Stop Them

Muscle cramping during exercise explained. Causes, prevention, and immediate remedies for cramps during training in Malaysia's hot climate.

Mid-set on your third round of leg presses, your calf seizes up like someone has stabbed it with an invisible knife. You grab it, gasping, wondering what you did wrong. Muscle cramps during exercise are painful, disruptive, and surprisingly common - especially when training in Malaysia's heat.

Here is what actually causes them and how to prevent them.

What Causes Exercise-Related Muscle Cramps?

The honest answer: we still do not fully understand why muscles cramp during exercise. But research points to two main theories, and both are probably right to varying degrees.

Theory 1: Electrolyte Depletion and Dehydration

The traditional explanation. When you sweat, you lose water and electrolytes - primarily sodium, but also potassium, magnesium, and calcium. These electrolytes are essential for proper nerve signalling and muscle contraction. When levels drop too low, muscles can fire involuntarily, causing cramps.

This theory has strong support in situations involving:

  • Prolonged exercise (over 60 minutes)
  • Excessive sweating - extremely relevant in Malaysia where humidity regularly exceeds 80%
  • Insufficient fluid intake during training
  • Sodium-depleted diets (rare in Malaysia, given how well-salted our food is)

In Malaysia's climate, you can lose 1-2 litres of sweat per hour during intense training. Each litre contains roughly 900-1,400mg of sodium, 200-300mg of potassium, and smaller amounts of magnesium and calcium. That is a lot of electrolytes leaving your body.

Theory 2: Altered Neuromuscular Control

More recent research suggests that cramps are primarily a neuromuscular phenomenon - specifically, fatigue-induced changes in how your nerves communicate with your muscles. When a muscle is fatigued, the normal feedback loops that prevent excessive contraction malfunction, and the muscle locks up.

This explains why:

  • Cramps often occur in muscles that are being worked hard, not all muscles simultaneously
  • Cramps are more common toward the end of exercise when fatigue is highest
  • Well-trained athletes who hydrate perfectly still get cramps sometimes
  • Cramps can occur in cool environments with minimal sweating

The Likely Reality

Both mechanisms contribute. Electrolyte imbalances lower the threshold at which fatigue-induced cramps occur. In other words, being dehydrated and electrolyte-depleted makes your muscles more susceptible to cramping when fatigued. In Malaysia's hot, humid environment, both factors are amplified.

How to Prevent Cramps During Training

Hydration Strategy

Do not wait until you are thirsty to drink. By the time you feel thirsty, you are already 1-2% dehydrated - enough to impair performance and increase cramping risk.

Before training: Drink 400-600ml of water in the two hours before your session. Your urine should be pale yellow (not clear - that means overhydrated, which dilutes electrolytes).

During training: Drink 200-300ml every 15-20 minutes. For sessions under 60 minutes in an air-conditioned gym, water is sufficient. For longer sessions or outdoor training, add electrolytes.

After training: Replace 150% of fluid lost during exercise. Weigh yourself before and after training - each kilogram lost equals one litre of sweat. If you lost 1kg, drink 1.5 litres in the two hours post-training.

Electrolyte Management

Sodium is king. For most cramp prevention, sodium is the primary electrolyte to replace. Here is how:

  • Add a pinch of salt (about 1/4 teaspoon = 600mg sodium) to your water bottle during long training sessions
  • Drink 100 Plus or a similar sports drink during sessions exceeding 60 minutes (provides sodium, potassium, and carbs)
  • Eat normally salted Malaysian food - our cuisine is generally well-salted, so dietary sodium is rarely a problem unless you are on a very strict low-sodium diet

Potassium: Eat a banana before training (422mg potassium) or drink coconut water afterward. Most Malaysian diets provide adequate potassium through fruits, vegetables, and legumes.

Magnesium: Often overlooked but important for muscle relaxation. Consider 200-400mg of magnesium glycinate daily, especially if you train frequently. Available at Guardian or Watsons for RM30-50.

Warm-Up and Stretching

Cold, unwarmed muscles cramp more easily. Spend 5-10 minutes warming up before intense exercise:

  • 3-5 minutes of light cardio (walking, cycling)
  • Dynamic stretching targeting the muscles you will train
  • 1-2 light sets of each exercise before loading up

Progressive Training

Cramps are more common when you suddenly increase training intensity or volume. If you have been doing 3 sets of leg press and jump to 5 sets, the extra fatigue can trigger cramping. Increase volume gradually - no more than 10-15% per week.

Conditioning

Better-conditioned muscles resist fatigue longer, which reduces cramping susceptibility. If your calves cramp regularly, they may simply lack endurance. Add calf raises (3 sets of 15-20 reps) twice per week to build muscular endurance in the area.

What to Do When a Cramp Strikes

Immediate Response

  1. Stop the exercise. Do not try to push through - the cramp will get worse.
  2. Gently stretch the cramping muscle. For a calf cramp, pull your toes toward your shin. For a hamstring cramp, straighten your leg and lean forward at the hips. For a quad cramp, pull your heel toward your glute.
  3. Apply pressure. Press firmly on the cramping muscle with your thumb and hold for 15-30 seconds. This can override the neurological signal causing the cramp.
  4. Massage the area. Once the acute cramp subsides, gentle massage helps relax the muscle.
  5. Sip electrolyte water. If dehydration contributed, start replacing fluids immediately.

The Pickle Juice Trick

Research from 2010 (Miller et al.) found that 70ml of pickle juice relieved cramps 45% faster than water. The proposed mechanism is not electrolyte replacement (the juice works too fast for absorption) but rather a neural reflex triggered by the strong sour/salty taste stimulating receptors in the mouth and throat, which sends signals to the spinal cord to inhibit the cramping muscle.

You probably do not have pickle juice at your gym. But the same effect has been reported with vinegar, mustard, and even strong chili - anything that creates a sharp sensory input. Keep a small bottle of apple cider vinegar in your gym bag as an emergency cramp remedy.

When Cramps Are a Warning Sign

Occasional cramps during hard training are normal. But frequent, severe, or unusual cramping patterns may indicate:

  • Chronic dehydration - common among Malaysians who drink insufficient water and rely on teh tarik and kopi
  • Mineral deficiency - magnesium or potassium deficiency, checkable via blood test
  • Medication side effects - statins, diuretics, and certain blood pressure medications increase cramping risk
  • Peripheral vascular disease - cramps in legs during walking that resolve with rest (intermittent claudication) - see a doctor
  • Nerve compression - cramps that follow a specific nerve pathway may indicate a pinched nerve in the spine

If you experience cramps frequently despite good hydration and electrolyte intake, consult a doctor. A basic blood panel (electrolytes, magnesium, calcium, renal function) can identify underlying issues.

Cramps are your body telling you something is wrong - usually dehydration, electrolyte imbalance, fatigue, or a combination of all three. Listen to the message, address the cause, and they will become a rare occurrence rather than a regular interruption.
